Via our forms: Last name, first name, email address, and phone number. This information is used exclusively to make appointments and follow up on your requests.<\/p>

Browsing data<\/strong>: We use Google Analytics to collect anonymous data (pages viewed, type of device) in order to improve the user experience on our site.<\/p>

Your consent is required for the collection of non-essential data.<\/p>

To request the withdrawal of your consent to the use of your data (subject to the legal obligations of the Ordre des dentistes du Qu\u00e9bec to retain records).<\/p>

As members of the Ordre des dentistes du Qu\u00e9bec (ODQ), we respect professional confidentiality and clinical record retention standards.<\/p>

8. Personal Information Collected<\/h3>

Centre Dentaire Acadie Sauv\u00e9 must collect the personal information necessary to provide dental care using the attached confidential medical-dental questionnaire, which is presented to you for your signature.<\/p>

The confidential medical-dental questionnaire is kept in your dental record, and your dental record is stored in a cabinet to which the public does not have access.<\/p>

Only healthcare staff will have access to your health information; administrative staff will only have access to the information necessary for the performance of their duties (billing, scheduling appointments, submitting claims to insurance companies).<\/p>

9. Your Rights<\/h3>
  • Right to access your dental record to review it<\/li>
  • Right to correct certain information<\/li>
  • Right to obtain a copy<\/li>
  • Right to withdraw your consent to the disclosure or use of information<\/li>
  • Right to restrict or refuse access<\/li>
  • Right to be informed if your information is disclosed outside of Quebec<\/li><\/ul>

    10. Retention Period for Personal Information<\/h3>

    Your dental record is retained for five years following the last entry or update to that record in accordance with the law, and at the end of this period, it will be destroyed in a manner that preserves its confidentiality.<\/p>
