As members of the Ordre des dentistes du Québec (ODQ), we respect professional confidentiality and clinical record retention standards.
Centre Dentaire Acadie Sauvé must collect the personal information necessary to provide dental care using the attached confidential medical-dental questionnaire, which is presented to you for your signature.
The confidential medical-dental questionnaire is kept in your dental record, and your dental record is stored in a cabinet to which the public does not have access.
Only healthcare staff will have access to your health information; administrative staff will only have access to the information necessary for the performance of their duties (billing, scheduling appointments, submitting claims to insurance companies).

Your dental record is retained for five years following the last entry or update to that record in accordance with the law, and at the end of this period, it will be destroyed in a manner that preserves its confidentiality.
This policy is reviewed periodically to ensure compliance with applicable laws.
